Welcome! All returning patients can tell you that you are going to have a great visit here at Visualeyes
Optometry. But we understand that finding a new doctor can sometimes give you a sense of both
excitement and fear at the same time. We hope this website has given you a little explanation of who
we are, and we are definitely looking forward to meeting you! Below is some information on what to
expect your first time with us. See you soon!

These forms will become a part of your medical record. The information it provides will improve our
ability to give you the best care possible.

items to bring
Please bring in your eyeglasses, contact lenses and solutions that you use, along with anything else
related to your eyes that you find necessary.

It is also good to bring in a list of medications that you are currently taking (pills and eyedrops), even if
they are not for eye-related problems. The list should include the name of the medication (trade name
or generic), the dosage (milligrams or percentage), and the number of times a day you use it. If you
have used any medications for the eyes in the past, that information is useful as well.
