Welcome to the Visualeyes Optometry Family!

We are honored that you have chosen Visualeyes Optometry to let us take care of your eyes. We hope you find your visit enjoyable. Through every moment that you are in our office, we are sure that we will make your eyes smile!

Before your visit there are a few items that we would like to bring to your attention:

Patient Forms

We have attached all of the forms that you will need to fill out at our office. You can print these out and complete them in the comfort of your own home. Just be sure to bring them with you on the day of your visit! These forms will become a part of your medical record, and the information you provide will improve our ability to give you the best care possible!

 

 

Items You Should Bring With You

  • Eyeglasses, Sunglasses, Contacts Lenses, Solutions, Eyedrops, and anything else related to your eyes that you find necessary.

  • Photo ID and Insurance Cards (Both Vision and Medical)!

Directions

We are located on the South/West corner of Van Nuys Blvd and Huston in the Sherman Oaks Medical Center. We are in the brick building closest to the hospital, on the ground floor. Suite 105.

You can enter the building through the back door, make a right turn, and we will be on your right!

​

Parking

There are three options for parking. 

1. You can park in the structure directly in back of the office. Unfortunately, we do not validate. The fee is $1 for every 20 minutes. 

2. You can park in the metered spaces on the street. 

3. There is a neighborhood right behind the office off of Huston. You can park there for no charge. It is a quick 1-2 minute walk to the office!
